Upper and Lower Back Pain and Sciatica

Different pathologies in various anatomical locations can contribute to back pain including conditions affecting the posterior and anterior columns of the spine.

Posterior Column

 The posterior column of the spine includes structures like the vertebrae, intervertebral discs, facet joints, ligaments, and muscles. Causes of back pain originating from the posterior column include:

1.   Muscle Strain: Overuse, poor posture, or sudden movements can strain the muscles and ligaments in the back.

2.   Facet Joint Arthritis: Degeneration of the facet joints can cause localized pain and stiffness.

3.   Herniated Discs: When a disc protrudes or ruptures, it can compress nearby nerves, causing pain.

4.   Spinal Stenosis: Narrowing of the spinal canal can compress the spinal cord or nerves, leading to pain.

5.   Spondylolisthesis: A condition where a vertebra slips out of place, potentially causing pain and nerve compression.

Anterior Column 

The anterior column primarily includes the vertebral bodies and the intervertebral discs located at the front of the spine. Causes of back pain originating from the anterior column include:

1.   Degenerative Disc Disease: Age-related wear and tear on the intervertebral discs can lead to pain and reduced flexibility.

2.   Osteoarthritis: Degeneration of the cartilage within the vertebral bodies can cause pain and inflammation.

3.   Compression Fractures: Osteoporosis or trauma can cause vertebral fractures, leading to significant pain.

4.   Discitis: Inflammation or infection of the intervertebral disc can cause severe pain.

5.   Ankylosing Spondylitis: A type of arthritis that causes inflammation in the vertebrae, leading to pain and stiffness.

Sciatica

Sciatica is characterized by pain that radiates along the path of the sciatic nerve, which extends from the lower back through the hips and buttocks and down each leg. Causes of sciatica include:

1.   Herniated Disc: A common cause where the disc compresses the sciatic nerve.

2.   Spinal Stenosis: Narrowing of the spinal canal in the lower back.

3.   Piriformis Syndrome: When the piriformis muscle irritates the sciatic nerve.

4.   Spondylolisthesis: Slippage of a vertebra can compress the sciatic nerve.
